You have two convenient options for completing these forms:
- Electronic Completion (Recommended):
- You can complete the forms directly on your computer or most smartphones.
- Open the attached PDF files using a compatible PDF viewer (such as Adobe Acrobat Reader).
- Fill in the required information electronically.
- Save the completed forms to your device.
- Email the saved forms back to us at records@myvwi.com
- Print and Pen Completion:
- If you prefer a traditional approach, you can print the forms.
- Use a pen to fill in all sections highlighted in yellow.
- Once completed, you have two choices:
- Scan and Email: Scan the filled-out forms and email them to records@myvwi.com
- Bring to Your Appointment: Bring the completed forms with you to your scheduled appointment.
